Driver's License Information

For those students who plan to apply for their driver’s permit, a “No Pass No Drive” School Compliance Verification: KRS 159.061 must be completed and signed by a parent prior to school approval.

A link for the form is below and should be completed and emailed to - martha.smith@boone.kyschools.us.  We require 24 hours to review and approve the form, prior to the students driver’s permit test.  The top of the form must include the student’s:

• Last Name
• First Name
• Middle Name (if applicable)
• Date of Birth
• Last 4 digits of their Social Security Number
• Parent signature
